Same as above, but without the instructions.
Short Form VA
Use only for unpublished works, or for single published photographs.
Group Registration Form
Use with Form VA above to register up to an entire year's worth of previously published photographs.
You only need to use this form when:

The photographs in the submission were NOT all published on the same date, and

Your submission sample of the photograph (the Deposit) does not indicate the particular date of publication of the photograph.
